Congestive heart failure (CHF) means the heart is not pumping blood as effectively as it should. When that happens, two things occur at the same time:

1. Blood Backs Up: Pressure Rises in the Veins
When the heart cannot pump well, blood returning to the heart through the veins has nowhere to go. Pressure rises in the veins and in the tiny blood vessels called capillaries. According to the laws of physics, when pressure inside the vessels increases, fluid is pushed out into the surrounding tissues.
Left-sided heart failure: Blood backs up toward the lungs. Fluid leaks into the lung tissue and air sacs, causing shortness of breath, coughing, and fatigue.
Right-sided heart failure: Blood backs up into the body. Fluid collects in the legs and ankles, called edema, and in the abdomen, called ascites. You might notice swelling, weight gain, and a feeling of fullness.
2. The Kidneys Hold On to Salt and Water
When the heart’s output is reduced, the kidneys receive less effective blood flow. The kidneys interpret this as a threat to survival—as if the body is “low on blood.” In response, they activate hormone systems that conserve fluid:
